Terms of Service

Reliable Respiratory dba Reliable Maternity Customer Agreement and Information 

Customer Agreement: 

  • Terms of Agreement and Medical Consent. Customer acknowledges receipt of all equipment and/or supplies listed on this delivery ticket, that he or she is under the care of his or her attending physician, and that Reliable Maternity/Reliable Respiratory shall not be liable for any act or omission undertaken at the direction or under the instruction of said physician. 

  • Medical Information Authorization.  Customer authorizes any holder of medical information relating to said Customer to release to Reliable Respiratory any record pertaining to his or her medical history, services rendered, or treatment.  Customer authorizes Reliable Respiratory to release Customer’s medical information to appropriate accreditation and insurance company personnel and other health care related organizations. 

  • Assignment of Insurance Benefits.  Customer authorizes any and all Medicare or other insurance benefits to be paid directly to Reliable Respiratory for any services or equipment furnished to Customer by Reliable Respiratory. 

  • Acknowledgments: 

    • Customer understands that certain equipment, supplies, and/or services may not be covered by his or her insurance company and may also be subject to a copay and/or a deductible.  Any amounts not paid or covered by Customer’s insurance company will become Customer’s personal financial responsibility, for which Reliable Respiratory may charge Customer’s credit card on file. 

    • The individual accepting delivery and/or any other person responsible for using or caring for any equipment and/or supplies has been thoroughly trained on the use, care and safety of the above equipment and/or supplies and has adequately re-demonstrated all procedures.  Reliable Respiratory shall not be liable for any acts or omissions of any Customer or other person in relation to the use of such equipment and/or supplies which violate these procedures. 

    • Customer acknowledges receipt of the Client Bill of Rights and Responsibilities, Notice of Privacy Practices, Equipment Owner’s Manual, and information regarding Medicare covered items under warranty. 

  • Default.  In the event that any invoice which is the financial responsibility of the Customer is not paid for a period of fourteen (14) days or more after becoming due, Reliable Respiratory may take action against the Customer to collect any amounts owed to it, including charging Customer’s credit card on file, without any further notice to Customer.  Reliable Respiratory may require Customer to return any equipment in its possession until such outstanding amounts are paid in full. 

  • Governing Law.  Any agreement between Reliable Respiratory and Customer shall be governed by the laws of the Commonwealth of Massachusetts, without application of its conflicts of laws principles. 

  • Severability.  Should any part of these Terms and Conditions, for any reason, be declared invalid or unenforceable, the invalidity or unenforceability of the particular provision shall not affect the other provisions, and these Terms and Conditions shall be construed in all respects as if such invalid or unenforceable provision had not been contained herein. 

  • I give permission for Reliable Respiratory, Inc. to use email in order to communicate with all relevant people involved in my diagnosis and treatment. 

 

Returns and Refund Policy

Breast Pumps are a personal hygiene product and as a result cannot be returned or exchanged if the box seal has been broken. If your pump is not performing as anticipated/expected, we recommend and encourage you to call the Breast Pump manufacturer for assistance. All special order items are final sale. 

 

To make a return 

  1. Message us through our contact form or give us a call at (888) 235-8355 and request a return. We will provide you with an RMA number; write this RMA number on the outside of the packaging you are using for your return, and include any documents associated with your order inside. 

  2. Ship your return to 1502 Boston Providence Turnpike, Suite 10, Norwood, MA 02062. 

  3. Ensure that your return is received at our facility within 30-days of the original delivery date. 

 

You are responsible for return shipping costs, including any shipping insurance purchased at your own discretion. We may provide a shipping label to you in certain circumstances, but this is not an acceptance of liability on behalf of Reliable Respiratory for any returns that are stolen, damaged, or lost in transit. Insurance coverage will not be included with any complementary return label. 

 

When shipping your return, you should select a shipment option that provides a delivery confirmation, so you can be sure that the package has reached our facility. Any refunds we issue will be based on the product received by our returns department. We will not offer a refund if we do not receive a product, or if the product is damaged or lost in transit. 

 

All returns will typically be processed within 48 hours of arriving at our facility. Any refunded amount will be credited to the original payment method. Shipping charges will not be refunded. 

 

Exchanges 

If you would like to make an exchange, please process a return for the original item, and place a new order for the product you would like. Separating these transactions allows for better tracking and management of your orders. 

 

Processing Errors 

If a processing error on our part necessitates a return or exchange, we will cover all costs associated with shipping, and we will do everything in our power to ensure that you are satisfied with purchase. Please contact one of our representatives via phone or through our Order Support form in order to process a return of this type. 

 

Warranty Claims 

We stand behind every product we sell. If you ever run into an issue with one of our products, give us a call and we’ll make it right. If we can’t resolve your issue with basic troubleshooting, we will process an exchange for your product or initiate a warranty claim on your behalf with the manufacturer of the product. Warranty claims are made at our sole discretion and are subject to terms and conditions. 

 

Shipping  

At Reliable Maternity, we endeavor to ship every order the day that it is received. Usually, orders that are placed prior to 11:00am EST will ship the same day. If your order requires expedited shipping, please call us at (888) 235-8355 so that we can find a solution that meets your needs. 

 

Orders are processed Monday through Friday. 

 

We do not fulfill orders on the following holidays: 

New Year's Day 

4th of July 

Martin Luther King Jr. Day 

Labor Day 

President's Day 

Thanksgiving and the day after Thanksgiving 

Memorial Day 

Christmas 

 

We do not fulfill orders to resale companies, and we reserve the right to cancel your order if we believe that you are buying on behalf of a re-seller. 

 

SHIPPING OPTIONS 

Reliable Maternity ships nationwide across the United States 

 

Free Standard Shipping 

Via Standard Shipping, your order should reach you within 4-7 business days of leaving our facility. 

 

2-Day Shipping 

2-Day Expedited Shipping is available for a flat fee of $25 on select orders. 

 

Priority Next Weekday Day Shipping 

Priority Next Weekday Shipping is available for a flat fee of $50 on select orders. 

 

POST-ORDER PROCESS 

We may contact you after your order has been placed in order to verify prescription information and to provide you with updates on your order. For this reason, please ensure that your contact information is accurate when placing your orders and when submitting any prescription information. Errors in contact information on the prescription upload form may lead to increases in processing time. 

 

Once your order is received, an order confirmation will be sent to your email address on record. This email simply constitutes a confirmation that your order has been received. It does not represent an offer to sell. We reserve the right to accept or decline your order for at any time and for any reason. If we cancel an order after you have already been billed, your account will quickly be refunded the amount billed. 

 

For most orders, you will receive a tracking number via email that you can use to track your order through the respective carrier’s website. You will also receive a shipment notification email when the order is picked up by the carrier from our facility. For more precise information regarding your order, or if you have any questions or concerns, please contact us by phone at (888) 235-8355 or through our contact form. 

 

High value orders ($250+) will require a signature upon delivery. 

 

For most orders, all items will ship at the same time. If any items are unable to be shipped at that time, the available items will be shipped first, followed by the remainder of the order as the items become available. 

 

Reliable Maternity/Reliable Respiratory is not responsible for any events that occur after the order leaves our facility, including late deliveries and/or any damage associated with the delivery of your order. 

 

MAKING CHANGES TO AN ORDER 

Once an order has been placed on our site, the fulfillment process begins immediately. If you need to cancel or correct an order, please call us as soon as possible at (888) 235-8355 or message us through our contact form. If an order has already been fulfilled, you will need to follow our standard return and exchange policies.
